Man Utd / Liverpool
England / Germany
Celtic / Rangers
Boca / River
Madrid / Barca

I don't think we'd ever understand the rivalry between Madrid and Barce given it involves one of one of the bloodiest European civil wars fought and the rivalry, not just between two cities, but two distinct cultures, languages and ethnicities. Celtic/Rangers would be close to that.

Boca vs River is standard issue poor people/rich people

I'd imagine a game like Serbia vs Bosnia or Serbia vs Croatia could be pretty 'tasty'.

It has to be Boca vs River, that rivalry surpasses any other in footballing terms and is literally a case of life and death.

Since the Hunt/Cech "incident" you'd have to put Reading/Chelsea in there, at least from Chelsea's point of view

Al-Ahly V El Zamalek (the Cairo derby)

Deaths used to be fairly standard practice.

But isn't Dinamo Zagreb vs Red Star the real nasty one?

Riot in the game around 1990, Zvonomir Boban kicking a policeman, tear gassing the fans.
Wonderful stuff.
